Rafael Antonio, Henriquez, 62, of Perth Amboy, passed away January 27, 2019, at Alameda Care Center surrounded by his family.

He was born in Santiago, Dominican Republic.

He is survived by his siblings, Lucia, Simon, Felix, Andres and Leoncito, and many nephews and nieces.

Visitation will be held on Wednesday, January 30, 2019 from 7-9 PM at Ana Maria Zevallos/Skrocki, 467 State Street, Perth Amboy, NJ 08861.

Burial will take place in Dominican Republic.
