Baby Jazleen Patino, 1 year old, of Perth Amboy, passed away on Friday, October 18, 2019 at Jersey Shore Medical Center.

Baby Jazleen leaves behind her parents, Kelvis Patino and Yinet Ramirez and her 4 siblings.

Visitation will be held on Monday, October 21, 2019 from 7-9 PM at Ana Maria Zevallos/Skrocki, Home for Funerals, 467 State Street, Perth Amboy, NJ 08861.

Funeral services on Tuesday, October 22, 2019 at 9 AM at the funeral home. Burial will follow in Our Lady of Hungary Cemetery.
