Agustin Rafael, Luna Polanco, 41, of Perth Amboy, passed away Wednesday, April 14, 2020 at Raritan Bay Medical Center in Perth Amboy.

He was born in La Vega, Dominican Republic.

He worked for Century Distribution at the macy’s warehouse as shipping clerk.

Agustin is survived by his mother, Maria Margarita Polanco and his companion, Juleisy Bautista.

Interment will be in Alpine Cemetery with a graveside service by Saint John Paul II Parish.
